Health Worker

The Community Health Worker (CHW) program provides training for participants to become Certified Community Health Workers (CCHW) through the State of Connecticut Department of Public Health.

CHWs act as liaisons between community members, healthcare, and social service providers. Possessing an in-depth understanding of their community’s experience, language, culture, and socioeconomic needs. CHWs also provide services, including outreach, advocacy, and care coordination. This program is available to residents of the 20-town region covered by The WorkPlace.

Learn about our amazing programs by attending an info session. There, you will have the opportunity to ask questions and learn about our different programs and vendors. Info sessions are available on Zoom.

Determine Eligibility

Carefully read the program requirements to determine your eligibility to participate. Once confirmed, a member of our staff will work with you to complete the application and begin the process of starting your career path journey.

Earn Your Credentials

Individuals completing the CHW program will receive a certificate of completion from the training provider.  After completing 1000 hours of work experience in the CHW space, individuals can apply for CHW state certification through the State of CT Department of Public Health.

People interested in enrolling in the Academy will need to complete an application, an ISP, and sign the HCA handbook. To be eligible for enrollment, you must be a resident of one of the following towns:
